New revenue channel

Get paid for the notes you’re already writing.

Doctor’s Note turns disability letters, sick-leave forms, and EDD paperwork into a self-serve storefront: patients request and pay up front, your team reviews an AI-drafted note, and the money lands in your account.

The math

Hours of correspondence, or seconds of review.

Most clinics write these letters for free and absorb days of phone tag doing it. Price the work, automate the intake, and the same volume becomes a predictable line of revenue — often tens to hundreds of thousands a year at group scale.

Zero back-and-forth. Structured intake collects diagnosis, limitations, and duration before it reaches you.
Paid when the note is sent. Credit card payments are made when the note request is approved — no invoices, no chasing.
Staff time back. Front desk stops relaying messages and starts approving completed work.

How it works

From request to payment, hands-free.

1
Patient makes a request

The request comes into your storefront — the patient picks a service, answers the medical questions, and pays up front.

2
AI generates the letter as a PDF

A HIPAA-compliant, AI-generated letter arrives in your queue as a PDF on your letterhead with your signature block.

3
Letter approval

Review the draft, edit inline, or approve as-is.

4
Auto-sent, payment made

On approval the letter is automatically sent to the patient as a secure download, and the payment settles to your account.

Set up whatever services you like.

Every kind of paperwork a patient asks for — set up the services you want to offer and set a price for each.

What your patients see

A page that looks like your practice, not a form. Verified credentials, your services, your prices — and a checkout that behaves like any modern store.

For organizations or individual physicians

Office managers run it. Doctors just sign.

For the whole clinic: organization accounts let one admin manage every provider’s storefront, services, and requests from a single login — with roles, per-provider access, and billing kept separate from clinical review.

Less work for doctors. The office manager runs everything — works for a solo physician, group, or enterprise.

Questions clinics ask first

Is charging for a doctor’s note allowed? +

Completing forms and letters is uncompensated administrative work in most jurisdictions, and practices routinely bill for it. You set the price, disclose it to the patient before they pay, and keep the receipt trail.

Does the AI decide anything clinical? +

No. It drafts language from the structured intake the patient submitted. Nothing is sent until a licensed provider reviews, edits, and signs it.

How long does setup take? +

Setup is quick: verify credentials, connect Stripe, add your services and letterhead. A guided checklist tracks what’s left before your page goes live.

Can we use our own letterhead and forms? +

Yes. Upload your letterhead and signature once and every generated note matches what your clinic sends today. Patient-supplied forms are handled as physical or online form requests.

What if we decline a request? +

You can reject any request. The patient is refunded through Stripe and the reason stays on the record. Clinical judgment never gets pre-committed by a payment.
